Apply for a student under 18

A student under 18 cannot open an account themselves, so a parent or guardian applies on their behalf and we review it before anything is set up.

EDUQIVA is opening to children aged 4 to 17. Applications through this form are accepted for ages 13 to 17 today; under 13 we cannot accept yet, because the verified parental consent that age requires is not finished. Adults register directly.

What you are agreeing to happen next, in this order:

An application. Sending this form puts a request in front of a person at EDUQIVA. It does not create an account, a class place, or any record of your child beyond the name and date of birth you type here.
A consent agreement for you to read and sign. If we can offer a place, we email it to you. Nothing is set up until you have signed it.
A short video call with you. So that we can be satisfied you are the child’s parent or guardian. A signed form proves that somebody typed a name into it; it does not prove whose child this is.

Nothing exists for your child until all three are done. You can tell us to stop at any point afterwards, and their classes end when you do.

We record which version of this notice was on your screen, and the moment it was shown, so that what you were given can be produced later.

Sending this creates a request, not an account. Nobody is enrolled and no login is made until a member of our team has reviewed it and you have signed the consent agreement. We ask for as little about your child as we can — a name and a date of birth — and the rest only once there is a place to offer.
